Violent Behaviour...

Violence or abuse towards any doctor or any member of staff will not be tolerated by the practice.

The police will be informed by the practice and removal from the practice list will be immediately requested.

Access to Records...

Patients may apply for access to their own medical records or may authorise a third party, such as a solicitor, to do so on their behalf.

Applications should be addressed to the Practice Manager and must be accompanied by written authorisation.

Out of Hours Care

Emergency Out of Hours care is provided by Gwen Healthcare NHS Trust. To contact the Out of Hours service, please ring 0845 600 1231.

Registering with the Practice

All patients wishing to register with this practice are asked to collect a registration form from reception.

Following completion of the form, it should be handed in to reception and an appointment made with the nurse for a New Patient Medical.

Patients can, if they wish, express a preference to register with a specific doctor at the practice, but freedom of choice is available should they wish to see any doctor in the practice other than the doctor they are registered with.

Repeat Prescriptions

If you require a repeat prescription, please hand in your repeat request to the surgery where you are registered and allow 48 working hours for this to be processed. We do not take requests for prescriptions over the telephone. Patients registered for dispensing services and living within the practice dispensing area are able to have their medicines dispensed at the Waunlwyd Surgery. Prescriptions will be processed in 48 hours.
